- Complete the forms assigned to you
- Complete your assigned forms and upload any labs from the past 3 to 6 months to the labs tab on the left side of your patient portal. If you do not have recent labs, we will send you a lab requisition
- Once everything is complete and your labs are uploaded or your lab results are back, we will reach out to you to schedule your consultation and take payment.
